HMB, or beta-hydroxy-beta-methylbutyrate, is a metabolite of leucine. It is marketed as a muscle-preserving supplement for hard training, dieting, and recovery. The evidence is more conditional than that marketing suggests.
What HMB is supposed to do
HMB may influence muscle-protein breakdown and recovery signalling. That is biologically plausible, especially when a person is exposed to unfamiliar exercise, energy restriction, illness, immobilisation, or another stressor.
A plausible anti-breakdown mechanism does not establish that HMB adds muscle or strength when a trained person is already eating and training adequately.
What the meta-analyses show
An early meta-analysis reported small lower-body strength benefits in previously untrained men but trivial effects in trained lifters and negligible body-composition changes. Later reviews are less encouraging for routine resistance training: a meta-analysis in trained and competitive athletes found no effect on strength or body composition, and another review in young subjects found no significant advantage for fat-free mass or strength.
The most defensible summary is therefore population-specific. HMB might be worth discussing in a period of severe stress or muscle loss, but the evidence does not support a general claim that it improves hypertrophy in a healthy, trained lifter.
When might it be relevant?
The possible use cases are not “everyone who wants gains.” They are situations in which preserving muscle is unusually important and diet or training is disrupted. Even there, the decision should account for the underlying cause, total protein, energy intake, medical status, and the fact that HMB is not a substitute for rehabilitation or treatment.
For ordinary training, protein-rich food, creatine, and a well-designed resistance program have higher priority. If budget is limited, HMB is easy to remove without sacrificing a proven foundation.
Dose and safety
Trials commonly use around 3 g per day, but a study dose is not a guarantee of efficacy. HMB-free acid and calcium HMB are different formulations, and results should not be assumed to transfer between every product.
Follow the label and ask a clinician if you have a medical condition or take medication. Do not stack several “anti-catabolic” products and then attribute a change to HMB.
Bottom line
HMB is a reasonable research question for specific high-stress or untrained contexts, but it is not a routine muscle-building requirement for trained athletes. The strongest direct reviews do not show reliable improvements in strength or body composition in trained lifters.
