Tongkat Ali for Lifters: What the Testosterone Evidence Shows

Tongkat ali (Eurycoma longifolia) is marketed as a natural testosterone booster. Some human studies have measured changes in testosterone or sexual-health outcomes, but the evidence is based on small, varied trials and specific extracts. A higher hormone value is not automatically better training, more muscle, or a diagnosis of low testosterone.

What the human evidence says

A 2022 systematic review and meta-analysis of clinical trials reported an increase in total testosterone in men receiving Eurycoma longifolia, while also stating that more research was needed before clinical use. The included studies were not all the same product, population, or outcome, so the pooled result should not be treated as a guaranteed response to a retail capsule. Read the meta-analysis.

A placebo-controlled study of a standardised water extract enrolled men aged 50–70 with total testosterone below the study’s threshold. It reported changes in total testosterone and some secondary measures, but it was a selected older population and the extract was a particular branded preparation. That is not evidence that a generic powder raises testosterone in a healthy young lifter or improves hypertrophy. Read the study.

A broader systematic review of “testosterone boosters” concluded that most ingredients lacked strong evidence and classified Eurycoma longifolia as possibly effective in selected populations rather than as a universal performance aid. See the review.

Hormone changes are not muscle outcomes

Testosterone is one part of a complex endocrine system. A short-term change in total testosterone may reflect timing, assay variation, binding proteins, illness, energy availability, or a study-specific intervention. It does not prove that a person has hypogonadism, that the training response will improve, or that the supplement is acting as anabolic therapy.

Symptoms such as low libido, erectile dysfunction, fatigue, low mood, or reduced performance have many possible causes. Do not use tongkat ali to self-diagnose or treat them. Product and safety problems

Research on one standardised extract cannot validate every root powder or online “test booster.” Labels, contaminants, adulteration, dose, and extraction methods vary. Independent testing is useful but does not turn an unproven outcome into a proven one.

Long-term safety and interactions are not established as well as marketing implies. People with hormone-sensitive conditions, liver or kidney disease, fertility concerns, psychiatric symptoms, medication use, or pregnancy should get professional advice. Stop and seek help for concerning symptoms.

If you are considering a trial

First check the fundamentals: sleep, energy availability, alcohol, training load, and whether a clinician has identified a genuine hormone problem. If you still choose to try a product, record the exact preparation, batch, dose, symptoms, and the outcome you care about. Do not stack multiple “testosterone boosters,” change medication, or infer efficacy from a pump or one blood test.

Bottom line

Tongkat ali may affect testosterone measures in some men, particularly in selected or lower-testosterone populations, but current evidence does not establish a reliable muscle, strength, or hypertrophy benefit for healthy lifters. It is not a natural replacement for testosterone therapy, a diagnosis, or the fundamentals of training and recovery.

Limits of the evidence
